People on sports teams call each other by their last names during the game sometimes. Neither forename nor surname can be ruled out.
In most cases yes, but in this league that my kids play in, they put their first names on the jerseys. This group too was in the "Under 12" division but all of the kids on my son's team atleast were in elementary school with only one being in 6th grade.
