Because I spend so much time in England I have adopted the mildly formal practice of always using the prefix "Mr." before the President's last name. Thus it was "Mr. Clinton", "Mr. Bush", and now "Mr. Obama". Alternatively, when speaking of the President I might say "The President". If I were to speak or write directly to the President I would address him as "Mr. President".
I may or may not like or approve of anyone who holds the office but he IS the President and I will show the respect and support due to the one who holds that office.
