Are you talking about the "truly horrifying" hazing or
anything that could possibly be considered hazing depending on the audience?
Individual members and chapters have gotten in trouble at the school, organization, and legal levels for doing seemingly harmless things. For instance, one legacy who doesn't get an invitation for membership may move on with his/her life whereas another legacy may accuse the chapter of trying to intimidate her/him and unfair treatment. Thus, members and chapters can get in trouble for hazing even if the intent wasn't malicious and wasn't to haze.
