It was a case of a young adult playing with a gun they thought was not loaded. He waved it around and held it to her head and it discharged - there was a bullet in the chamber.

Shock and Sorrow all around. I'm posting this only to remind everyone that guns are dangerous and should always be considered to be loaded.
