And to be clear you don't always wait until the pre-initiation vote to cut pledges. If you've encouraged pledges to turn things around strongly and they are still far and away the worst pledge, you can take a vote to send 'em off.

In my 4 years I think we've probably cut 8-10 pledges and a number more have left after strong encouragement to do so that would have been cut had they stuck around.

And to the bolded part - this is an example of why the number of pledges cut may not be as great a number as it could be. And it is often the best scenario for both parties. A pledge that is not going to make it is encouraged to drop (resign) which in turn allows him to save face and perhaps pursue other chapters if he wants.

The ex-boyfriend of one of my roommates in college only got about halfway through the pledge semester before being asked to leave his pledge class. It may have been for the "quoted" reasons above, or maybe those are just the qualities I personally thought he had. He honestly wouldn't have been a good fit in that chapter, and I think generally the fraternities know what and how many they are looking for.
