This differs so much from campus to campus, and sorority to sorority, it would be almost impossible to answer. We have formal invitations to accept a bid, but I know other chapters do it differently.

I think most of the NPC sororities have formalized card type invites that they present to the women along with some sort of gift... other than that it I really dont' think there's any standard of what to do... usually my chapter has a spaghetti dinner and watches a movie or something chill where we can relax and have fun.... after a week of rush the last thing you want to do is have something super stressful/formal to do on bid day too....

Write something like "Sorority Name cordially extends an invitation to membership to Girl's Name. We have chosen you because you exemplify Your Virtues." Be sure to include a deadline for accepting as well.

At my school, the rushees are just given a piece of paper that says something like
Dear _______________,
The following organization(s) have chosen to extend you a bid.... then it lists the names of all of the sororities, and they check off the ones that have given that rushee a bid. Underneath that, is a slip that they have to tear off that says if they're accepting their bid, and if so, to which sorority. During bid acceptance, they go to the student center where all of the sororities set up tables and hand in their bid card to the sorority that they're going to pledge. That's when all of the sororities give them a formal bid. Usually we write stuff like Congratulations on accepting your bid. The mandatory anti-hazing meeting is on such and such a date. If you have any questions, please contact your pledge mom at such and such an extension. Congratulations and have fun! etc...
