The majority of the final episodes of Evangelion are pretty disturbing, anyway...once Misato finds Lilith, Askua gets mind raped, and the entrance of Kaoru.
Schindler's List was incredibly disturbing for me as well. Mainly because the events actually happened. And while I was a bit disturbed by Silence of the Lambs, etc., List still tops it for me. But it's still a really good movie.
