That's not to suggest a woman with a particularly large chest wouldn't also experience those as problems i.e. that 'people look (gawk) at them' of course, nor that a considerably large chest would not be disproportionate to a smaller frame. Often these can be secondary concerns though.

It's just slightly surprising that that reasoning (the latter two) is being thrown around in isolation. Is it a difference between a reduction being for medical reasons and purely cosmetic reasons?
The latter would of course exclude cases where the breast size though not so significant as to cause back problems does cause serious esteem or depression issues and accordingly suggests reduction surgery as an appropriate intervention.
In order for health insurance to cover the cost of cosmetic surgery, a patient often needs a medical reason such as back pain for breast reduction or deviated septum for a nose job. Many health insurance policies in the U.S.A. do not cover surgery for purely cosmetic reasons.
