I agree that I would be too much of a mess to eat and that I'd order the most expensive things possible, but those two things aside this is what I'd want:
appetizer - mozzarella sticks w/ marinara sauce
entree - filet mingon wrapped in bacon cooked medium rare with home made mashed potatoes and grilled zucchini
dessert - a HUGE piece of chocolate cake from a very nice bakery
drink - the strongest moonshine there is (so that I would not be conscious for what was about to follow)

Appetizer: A big plate of Indian appetizers - samosas, pakoras, dosa, etc.
Entree: Surf and turf: filet mignon medium rare and a steamed lobster. Corn on the cob and new potatoes.
Dessert: A slice of flourless chocolate cake a la mode.
Drink: If I'm allowed to have alcohol, I'd have a good bottle of Cabernet. If not, sparkling water.

Heck no money spared.... who cares if i eat it all but order anything i want :
Appetizer: outback blooming onion and a olive garden articoke spinach dip
Dinner: Lonestar large primerib cut with creamy horseraddish and a large sweet potato and brown sugar and butter and steamed zuchini and squash
dessert: either crem bulee or banana cream pie
beverage: jack and coke x 5 glasses so i am so knocked out i dont know i was going

For a fantasy last meal I would have nectar and ambrosia, the food and drink of the Greek gods. Since they confer immortality when they came to execute me I would just laugh and say, Lotsa luck guys!

appetizer-Some garlic very hot cajun wings from Wings Plus, the 20 piece with bleu cheese, no celery
entree-slow smoked barbequed country beef ribs, extra cripy edges, with some REAL cheese and macaroni, not the box garbage, some collard greens with turnips, some garlic cheddar baked potatoes, a cheesy enchilada form my brother with olives and scallions, and a salad from the Olive Garden with extra pepperoncinis and olives
dessert -Breyer's Cherry Vanilla ice cream, Haagen-Das chocolate sorbet, and Edy's Tropical sorbet, all in a banana split
drink-my own concotion consisting of frozen pineapple chunks marinated in Hypnotiq, put in the blender with extra hypnotic, pineapple juice and colada mix.

appetizer- cheese sticks and marinara (from a local ale house)
entree- smoked salmon, garlic mashed potato, corn on the cob, salad with ranch dressing
dessert- as many orange sherbert/vanilla ice cream cups I can eat
drink- Cosmos if alcoholic, plain ol' Coke if not

first - crab cakes and chili's southwestern eggrolls
entree- 5 lbs. of alaskin king crab legs
desert- half sheet of greman chocolate cake with out coconuts and orange sherbert ice cream
drink - tropicana orange juice mixed with vanilla ice cream and whip cream

my first choice:
appetizer
skip dinner and go straight to dessert
dessert
if i can't have that then.....
appetizer: chicken strips, cheese sticks
dinner: fried rice, sweet and sour chicken, bbq buffalo wings, bbq ribs
dessert: strawberry cheesecake
drink: water will do

Dinner- Jimenetti's Rack of Ribs w/Jack Daniel's rub, Applebee's mashed potatoes, China 1's rolls & noodles
Dessert- Cheescake Factory's Reese Peanut Butter Cup Cheesecake..the entire cake & Shake Shoppe Butterfinger shake
