Not necessarily. I'm related to my uncle by marriage (in other words, my aunt is my blood relative, as she's my mother's sister). My uncle has a sister, who has a child. Therefore, that child would be my uncle's niece, my aunt's niece, but would have no relation to me. Clear as mud? Here's a diagram:
