Basically it's just an idea that shows up on the internet and gets passed along into the collective conciousness. A GC meme would be DITY for example. A Sienfeld meme might be "not that there's anything wrong with that"
Visual memes include the Haters gonna Hate pics, and even just the idea that cats talk in kitty pigdin.
