Basically:

About 7 years ago she got mixed up in a religious group that told her that she could not be a good Christian and also a member of her NPHC. They made her sign something that she "thought" terminated her affiliation with AKA.

A year later she realized that the religious group was not all good (including, according to her, the leader who was also NPHC) so she got out.

Now she wants to know if she is still a member of AKA, as she didn't know if that piece of paper had ever been mailed, or really what it was that she signed. She claimed she was too embarrassed to ask her line sisters. She was hesitating to call HQ.

That is pretty much it in a nutshell.
