I go to Texas, most of the systems are pretty similar.
1. You are probably going to make you go to each house, at least for a little while. Makes it fair....each house gets to see ALL of the rushees, etc. Usually it's a week long or so. Houses will have stuff going on for your entertainment, food, etc.
2. Make a good impression. Usually if a house likes you you will get invited back for more private events later on in rush. They'll get in contact with you. I've never heard of having to fill out a form for each house. If they like you, you'll sign a bid card to whichever house you choose.
3. Anything really. Just be talkative, act like a gentleman, dress appropriately......have fun. Ask about philanthropy, big party events......just normal conversation. Don't be shy.
