The way I read the article in Soap Opera Digest was that Zarf had already had the surgery (from female to male) and therefore at one point being a female "he" was attracted to Bianca. Although, now that I think about it, it makes much more sense for Zarf to be a "female" stuck in a "males" body.
I guess I should start watching AMC again.
