As much as I love our website, unfortunately, much of it is member's only. I did find the form in a word document, so if anyone would like it, they can PM me.

She ripped off so many people, her score was down in the 70's, so she opened the second account, sold some things to herself, and gave herself great feedback until it was up to the 90's. Then, she closed stephmnelson, and started using bamfranger99 exclusively.
