Ok, here's my view since its my favorite movie:
1. I always thought that old woman was Shug's mom.
2. I always thought the girl singing was Shug's daughter who the grandpa and grandma were raising since Shug was in the world.
3. Woman playing the piano...I have not idea. Probably just a great pianist
