After a girl goes through Pledge Service and gets a sister-mother, we have both sister-mothers and daughters fill out an information sheet. It's a page found in our New Member Manual. Some of it looks like the Recruitment Info Card, but there's a bunch of other things we ask. Here's what ours looks like:

- School Address/Phone
- Home Address/Phone
- Brothers & Sisters
- Birthday
- Year in School/Major
- Extra-curricular activities
- CLASS SCHEDULE
- Favorite Color, Soap Opera, Soft Drink, College Hangout, Movie, Music, Movie Star, Pizza Topping, Junk Food, TV Show, Holiday, Hobby, Book, Ice Cream, Magazine, Candy
After revealing the moms and daughters trade their info.
