Pledge Forms

My sorority wants to ask our "pledges" to fill out some sort of form about themselves during pledge week, kinda like an application. Anyone have any ideas on how to start an application?

We really don't know what kinda of applications. We would hand them out at our last rush event!

After a girl goes through Pledge Service and gets a sister-mother, we have both sister-mothers and daughters fill out an information sheet. It's a page found in our New Member Manual. Some of it looks like the Recruitment Info Card, but there's a bunch of other things we ask. Here's what ours looks like:

Sister-Mother/Sister-Daughter Information
  • Full Name
  • School Address/Phone
  • Home Address/Phone
  • Brothers & Sisters
  • Birthday
  • Year in School/Major
  • Extra-curricular activities
  • CLASS SCHEDULE
  • Favorite Color, Soap Opera, Soft Drink, College Hangout, Movie, Music, Movie Star, Pizza Topping, Junk Food, TV Show, Holiday, Hobby, Book, Ice Cream, Magazine, Candy

After revealing the moms and daughters trade their info.

I'm confused, do you want them to fill these out before or after they have their bids? I wouldn't hand them out at the last rush event if not everyone there is guaranteed to get a bid. Some womem might take it the wrong way.
