I struggled with this form as well. I had a very difficult time adding text boxes in Adobe Acrobat because the form was so tight. I ended up recreating the form in Word; I enlarged the form so it is the full page. This made it much easier to type the Potential Member's information (especially when she has lots of activities or volunteer time).
