MIS Form
 

Is anyone able to to convert the MIS form that is only to a Word doc? I have a lot I want to say about a girl and it would be so much nicer to type it up.

Thank you. If the form look is lost, that's fine. I can play with it in Word to make it look right. :)

When you export at Word doc, it just takes a JPG of the form and places it in a Word document.

If you have full version of Acrobat, you can add text boxes to 'type' in the data, that's what I do.

I only have Acrobat Reader or else I would. Thanks anyway though.

I struggled with this form as well. I had a very difficult time adding text boxes in Adobe Acrobat because the form was so tight. I ended up recreating the form in Word; I enlarged the form so it is the full page. This made it much easier to type the Potential Member's information (especially when she has lots of activities or volunteer time).
