even if your supplier sends you brocoli with a snake head, you have enough staff/pressence of mind yourself to not bring it to the customer. That's the prep cook, line cook, kitchen mgr/exec. chef, server/waiter/waitress, general mgr., FOH mgr., and possibly a food runner that would have/should have seen that.

According to the reports, lab tests determined that the snake head had not been cooked. They now believe that someone added it to the plate after the meal had been cooked and are now investigating the possibility of intentional tampering.
