How it is run and how it is setup encompass MANY things. It's like starting anything else. You need to have a mission and then everything else falls into place. Most of your larger US fraternities share the goal of helping to build men of good character, the fraternity has its ideal member. For example SAE has the "True Gentleman" and Sigma Nu's ideal brother practices "Love, Honor and Truth". You can find more about that on an organization's website.
From there you need to establish your business side of things, write a constitution, bylaws, etc. Decide what kind of meetings you will have and what the inward and outward goals of your organization are.. If it takes money to do these things you may want to charge your members dues.
I'll stop here, but there's soo much more involved in starting a fraternity. I did a great deal to start the chapter of Sigma Nu at my school and in my humble opinion we're the best fraternity around (and if you ask anyone from any other fraternity they are also!).
In truth there are far too many intangibles and different ways of looking at things to truly say who the "best" fraternity in the US is. If you have any specific questions though, there are several of us out here that have helped found our respective chapters so ask and ye shall be answered.
