recommendations for incoming freshmen

Does anyone know if UT Knox requires specific recommendation forms from alums, and where I might get one if they do? I have been asked to write a rec for a girl (ZTA) and I can't seem to get any definite info. If anyone knows anything about this I would greatly appreciate the info!! I really don't want to let this girl down. Thanks!!
