Padma was put into Ravenclaw (while her twin sister Parvati is in Gryffindor). The twins famously accompanied Harry and Ron to the Yule Ball.

He was entered by who we originally thought was Mad-Eye Moody but turned out to be Barty Crouch Jr. after the effects of the Polyjuice Potion wore out. He probably put a Confundus charm on the goblet to make it forget that only 3 names were supposed to come out of it.
