Shadokat,
the Herald article stated that the van had a poor exhaust system which caused the carbon monoxide poisioning. From what I can figure they were probably idling the car for a while and the exhaust got in to the van. Kindof like they tell you if you get stuck in a snow drift to not turn on the car for heat - the exhaust will get in the car.
Please everyone if you have an older or poorly maintained car, get your exhaust system checked so another tragedy is avoided.
