At Illinois girls we wore other letters all the time. It was "cool" because it meant you had friends in that fraternity. Usually they were just loaners - let me wear your sweatshirt for while and you can wear an AOII hat or something. It didn't designate being a sweetheart or anything.
ETA: It was also "cool" to have paddles from other fraternities and sororities that you had been given as gifts.

Only one group does sweethearts on my campus, although I think our local may have them as well. They do get a set of letters to wear, but it's usually just a t-shirt. Two of my sisters are sweethearts, but neither wear the shirts often. I'm not really aware of any rules, traditions, etc., surrounding the wearing of sweetheart letters.

That is our policy as well. Our letters themselves can only be worn by initiated members, but the english names can be worn by pledges, parents, husbands, etc.

most of the fraternities [sigep, pikapp, pike] on my campus give their sweethearts a jersey with their colors as well as stitched letters on them
only sammy and fiji give their sweethearts jerseys with "sammy" and fiji" spelled out...
being a sammy sweetheart, i respect the fact that i dont get "letters" although i am a chosen representative, b/c to be honest, those letters mean NOTHING compared to what mine mean to me. don't get me wrong it's an HONOR, but i still feel uncomfortable b/c i'm so used to wearing MY letters... [i'm weird, i know]
anywho...
some sororities on my campus also award fraternity men with a title equivalent to sweetheart, but none of them give out their letters [axid- jersey with "xi man", phi sigma sigma- jersey with "mr phisignificant", aopi- jersey with "IIOA", dphie- jersey with HIS organization's letters in OUR colors and "campus man" stitched on the back]
there are other organizations, but i can't recall what they do...

We let our Keymen wear the jerseys with the word kappa in script.

even men who are not part of a greek organization run for the title, but at our campus everyone is part of SOMETHING, and they represent it proudly.. whether they are representing the student alumni association [SAA], panther rage [PR], student housing association [SHA], football players [FIU - florida international university] or even a cheerleader - yea we have male cheerleaders [FIU]....

random: last night was my chapter's 15th annual campus man pageant [where the next "sweetheart" is crowned]... the top three were a pike, fiji, and a sammy... our new 2006 campus man is a SAMMY! ♥
ps: they raised over $1500 for the Cystic Fibrosis Foundation! ::i'm so proud of my girls!!::
