It all depends on the insurance provider and plan. Ours covers all reductions, because they can be either preventive medicine or relieving the various problems that come with having very large breasts. I have friends, from the children of physicians to Gap employees, who have had their insurance cover their reductions.

I had plastic surgery when I was younger after an accident. It wasn't disfiguring, but it had the potential to be as I got older. I plan on getting implants, 'cause I think some serious weight loss around boob-growing age impeded what would have been natural growth (as evidenced by female relatives on both sides of the family). We have private insurance, but a family member who works for Uncle Sam had the cost of the operation covered by Tricare (not the implants, though).
