"Y'all" is used when speaking directly to one person. "All Y'all" is used when addressing a group. Yes, there is a difference.

I've heard people use it as a standard pronoun no matter the number on so many occasions.

There's also "y'alls" for plural and "all y'alls" when adding emphasis.
