Normally, the Excedrine Migraine helps me also but it has become inadequate. My doc put me on Imitrex but I went through them so fast (2 packs of 9 pills in less than two weeks!) that she said to go back to the EM until I see the headache specialist.
Unfortunately, I was not able to get an appointment until the first week of May so I suffer in the meantime.

I get them really bad around my cycle also, my doctor says they trigger them. Look into cluster headaches you may find you symptons are very similiar to those types of migraines. I have gone through 3 packs of imitrex in 3 weeks (9 pills) $30 a presciption.
