According to the Washington Post and writer Michael Dobbs:
Rather's documents have a heading saying they were FAXed from Kinkos in Abilene, TX.
Abilene is 21 miles from the home of retired (since 1998) TX Natl. Guard officer Bill Burkett. He says he suffered a nervous breakdown and was hospitalized for depression after he left the guard.
He has a long-running dispute with the guard regarding medical benefits concerning a tropical disease after an assignment in Panama.
Burkett earlier accused Bush aides of ordering destruction of portions of Bush's National Guard records, and said he overheard a spring 1997 phone conversation during Bush's 1997 governor campaign. The people Burkett named as overheard strongly denied the allegations.

To summarize (mine, not the Wash Post): If there was a way to examine the hard disk at the self-service computer at Kinkos, you'd probably find the forged documents.
The forgery was done my a disgruntled NG officer, who has a history of mental illness and a grudge against the administration which has denied him benefits.
