Its been that way all of my life. When I was growing up in Brooklyn, and we were heading into Manhattan, we would say that we were "going to the City," even though Brooklyn is part of the same municipality.

Also, people who live in Manhattan don't identify with living in Manhattan. We identify with areas and neighborhoods. For example, Manhattan south of 23rd St. is "Downtown." Downtown has a different culture than Uptown. So many people (myself included) identify more with Downtown than another area. Also, Downtown has many neighborhoods including Greenwich Village, SoHo, TriBeCa, Chinatown, Battery Park City (which is all the way down, but culturally more like Uptown) and more.
