I voted in Maryland before I came to work today. I had to use a provisional ballot because I was not transferred from my prior location (in 2003, I moved to another part of the same county).

However, there was no big deal about the provisional ballot. The poll worker looked and saw that I was registered last year so I had to fill out a registration with my new info (even though I did that when I changed my drivers license last year) and complete a paper ballot. The entire package was placed in a one-way day-glo orange bag marked Provisional Ballots.

I was in the polling place for a total of about 45 minutes (30min in line) and I was the only person during that time who had to cast a provisional ballot.
