Bradley Bayou does in his Halston line up to size 20. Chanel does up to 20, so does Bill Blass. I've seen Carolina Herrera, Akris and Oscar de la Renta do size 18s, and there's a guy named Richard Metzger who did stuff for Oprah. RM specializes in sizes 12-22; I heard he was going to be on a reality TV show to find a new hot fashion designer.
