Black Rose Formal - You ask your date out by delivering them a black rose (same way as the symbol of death in Sicily - it was so gothic!)
Renaissance Fair-style dance
It's kind of trite, because it was in a movie, but I really liked the Prom theme in Never Been Kissed -- "Meant For Each Other - Famous Couples Throughout History"
