I would say it's just very severe body image issues. Some girls are brought up to believe, or are told by insensitive ex-boyfriends (usually the ones who don't want to do it) that their genetalia are dirty or unattractive.
I mean, bad oral sex is kind of like bad pizza, it's still pretty good.
Unless the guy is using his teeth in the wrong places or something.
