Suspended can also be the result of a deal -- lawyer agrees to suspension to forgo a hearing and the chance of disbarment. Kind of a plea deal.
In my experience at least, suspensions for CLE deficits and failure to pay bar dues are rare, because those things are easy to rectify. More often, a suspension is the result of a first time, serious ethics violation.
Lawyers can also receive censures, which are essentially warnings that don't affect licensure status.
