At work, we obtained a silver sequined beret (won't get into how) which my team has named the "Beret of Shame" (yes like the "cone of shame" a la Up!). If you mess up and say for example, attach the wrong document in an email or send something out with a funny typo (basically, non-serious screw ups) you get the beret of shame for the day.
