on the recruitment form you should answer "yes" to the question "have you been thru recruitment". if there is space to add it, you can add that you withdrew from recruitment due to illness.
as to what to say to sorority members, i really don't think that they will ask you if you participated in recruitment at your former school, but if they do, you can just say something to the effect of,"i did participate in recruitment thru the first (or second or third) round, but i was really sick and had to withdraw."
