I know we've used customink.com. And it's pretty good, as far as online t-shirt design goes. They're kind of overpriced, though.
I find that it's easier, usually cheaper, and faster to find a good local place, and just have them fill out the quick form to be official with your GLO. There was an awesome local place I used in Minneapolis, for instance, that could get me ~100 shirts done in less than 5 business days, and for a way cheaper price, and the shipping then is $0.
