Inter-Zeta transfer - forms needed?

When a brother wants to transfer from his own Zeta to another Zeta (when he changes schools) what is/are the form(s) needed. I recall that it used to be a Form 5 but that was years ago. I believe that there would also be a roster change form as well.
Our High Gamma manual is a copy of a PDF and the form shown just isn't very distinct.
Does anyone have the approrpiate forms and could you fax them to me at 800-760-0962?
