About your grandma, I am so sorry that she was put into a home. I would definitely get some pictures together for her, and put them in a large frame (one for the wall if they allow that) - nothing grandparents like more than talking about their grandkids! If possible, add names on each picture - it will help her and others as they come to visit her and chat about the pictures.
Another idea - depending on her mental ability - is to make her an "Everyday I Love You" jar. Just cut strips of paper out, write a quote, poem, a fun memory with her, etc. and then fold them up. Put them all in the jar (can be a candy jar, small plastic container, etc.) and tell her that every day, she can pull one out and read it and think of you.
