House Bill Payments with Loan $$$

Delts,

We are looking for a form that our students/Brothers
could complete if they are attaining student aid or a loan to
pay their House Bills.

We would like to have them sign off as well as receive
confirmation from our University Financial Office....

Does anyone have a form that offers that?
