He was entered by who we originally thought was Mad-Eye Moody but turned out to be Barty Crouch Jr. after the effects of the Polyjuice Potion wore out. He probably put a Confundus charm on the goblet to make it forget that only 3 names were supposed to come out of it.

Actually, Barty Crouch Sr. is NOT the head of the Goblin Liason office.

The word Crucio means torment.
