Thank you! Only problem is there is no hard copy form (that I can locate) anymore. Can't find my most recent Anchora which historically has one inside. Hopefully EO will direct me. Glad the timing is not quite as urgent as I thought.

You should have one in the mail already or will in a couple of days. If not, pm me.

There is a hard copy form on the website. You have the option to print it out and mail it or save it as a PDF and and email to the chapter in question. Arkansas has requested that they be mailed to the Executives Offices. I will PM you the link. I have done it both ways in the past few months.

Oh, my gosh they have changed it on the website in the past week! I just did 2 last week the way I stated in the first paragraph!

You can also write a letter, touching on the same 5 points and info that are on the sponsor form, and it will count as a sponsor form.
